In collaboration with Fair Haven Community Health Center and Yale New Haven Hospital, the Clifford Beers Clinic is pursuing a Center for Medicare & Medicaid Innovation Award to further advance their integrated model of care for children with serious behavioral health challenges. This grant will support the long-term sustainability of taking to scale this integrated care model.
