When one health care provider organizes at patient’s care activities across providers and services, that’s care coordination. Despite its benefit to health consumers, the current system remains fragmented, requiring patients to put the pieces of their care together themselves. The gaps that result have a disproportionate impact on racial and ethnic minorities, who are then not linked to effective care.

This grant to the Office of Community Child Health will support the Hartford Area Care Coordination Collaborative formalizing their initiative into a coordination utility where medical practices, agencies, and state departments can outsource their coordination activities, providing better support for the families and children they care for.
