Last year (2011), the Corporation for Supportive Housing (CSH) received a grant to create a planning process and develop a model for integrating supportive housing services with federally qualified health centers for the homeless in Connecticut. This year, CSH is the recipient of a federal Social Innovation Fund (SIF) grant and has chosen four demonstration sites, of which one is Connecticut, to further pilot their integrated model. The learnings from the Connecticut Health Foundation’s (CT Health) grant will inform the work of the national demonstration.

CT Health’s contribution will help CSH meet a portion of the dollar-for-dollar matching requirement embedded in the SIF grant.
