When attempting to eliminate racial and ethnic health disparities, it’s important to factor in all of the systems that contribute to the health of an individual. Creating affordable, supportive housing services for the homeless helps address a key social determinant of health. This grant from CT Health will help the Corporation for Supportive Housing to create a planning process and develop a model for integrating supportive housing services for formerly homeless individuals and families with primary care/medical home services of partner federally qualified health centers (FQHCs) in New Haven and Hartford. The planning process will involve reviewing research and promising practices, establishing partnerships, refining the program model, and designing implementation, evaluation, and financing plans.
