MISSION STATEMENTThe mission of the Connecticut Health Foundation (CT Health) is to improve the health status of people in Connecticut. With this mission, CT Health aims to make qualitative and measurable differences in the health and well-being of individuals and families. The foundation actively strives to serve the unmet needs of the state and its communities and to be responsive to unserved and underserved populations.To fulfill its mission, CT Health pursues the following goals: - Increase access to quality and affordable health services for all Connecticut residents
- Promote wellness, prevention of disease, and active management of chronic illnesses and conditions
- Encourage improvement of health outcomes and wise use of health care resources
In pursuing these goals, the foundation supports and advances models of care, policy initiatives, education programs and research. CT Health recognizes the importance of individual and organizational cultural and linguistic competence in order to improve health outcomes of those who have been historically ignored and/or underserved in the health care system. In addition, the foundation embraces collaboration with other organizations that share a common vision. Vision CT Health is a model of philanthropy that makes a difference in the quality of life for individuals and families in Connecticut. It serves as a catalyst, building consensus and coalitions to create sustainable improvements in health services so that people can pursue and enjoy optimum health in body, mind and spirit. CT Health believes strongly that "health" includes mental, physical and spiritual well-being, and that the health of individuals, families and communities are intertwined. Further, no matter how good existing health care systems may appear to be, they are not much help if they are not truly accessible. Supporting programs to improve health and access to health care means going beyond a medical model and looking at ways to support people to be the best they can be.
