City of Hartford Department of Health and Human Services (DHHS) was awarded a one-year President’s Discretionary Grant to bring together health care safety net site leaders to identify creative solutions and resources to improve health care delivery to Greater Hartford’s underserved communities. The group will develop a roadmap to develop their initiative, Safety Net Without Walls.
