Connecticut Children’s Medical Center – Office of Community Child Health was awarded a one year grant to support the Hartford Area Care Coordination Collaborative to expand their ‘shared resource, central utility’ model to New Haven, Norwich, Stamford and Waterbury. The ‘central utility’ model promises efficiency in the delivery of appropriate health care and other services – ensuring that children and their families get coordinated care services, and managing transitions in care among providers and community agencies.
