Fiscal Agent: The Connecticut Healthcare Research and Education Foundation. The Connecticut Hospital Association, was awarded a one year grant to develop capacity within hospitals and collaborating provider and community organizations to identify and address patients’ social determinants of health at the point of care to improve health outcomes on both an individual and population level. This project will specifically address the needs of Asthma patients.
