Hartford HealthCare was awarded a one-year grant to test a new model of integrated care in eight pilot sites across Connecticut. The model embeds behavioral health clinicians in primary care offices to improve the health outcomes of high risk patients who suffer from multiple physical illnesses and related behavioral health conditions.
