Hartford Healthcare HHC (Hartford, Statewide) was awarded a one year $100,000 grant for the second year of their Primary Care Behavioral Health Integration Project. HHC plans to continue to expand its primary care behavioral health collaborative care model from five pilot sites to seven. This model embeds behavioral health clinicians in HHC primary care offices, using a team-based model and community case manager follow up. Year two funding will support expansion of this model to New Britain and Windham.
