United Connecticut Action for Neighborhoods

The United Connecticut Action for Neighborhood’s (UCAN) Caring Families Coalition (CFC) proposes to spend 2013 engaging Hartford families and consumer advocates in systems planning, design, and advocacy with the Department of Social Services and Access Health CT in the implementation of health reform. Additionally, CFC will train its community members in what the state Medicaid program’s Person Centered Medical Home Initiative (PCMH) means for them as patients now expected to be active partners in an enhanced model of primary care delivery.

This grant provides funding to train a base of CFC members to participate in the planning, implementation, and improvement of the PCMH model, develop culturally appropriate and sensitive educational materials for Medicaid enrollees in advocating for their health care, and promote the voice of consumers in the decisions of the Access Health CT Board.
