Since CT Health’s founding, our grantmaking, policy and leadership work have been driven by what we call our theory of change. Our recent transition away from categorical funding is just one more way we’re bringing this catalytic concept to life, even on an internal level. But what is systems change? CT Health asks those seeking our funding and partnership to tell us about what systems need change in Connecticut. But understanding and discussing systems change can be challenging. Systems change definitions are typically academic and jargon-y. We hope this infographic, which offers a new way to think about systems change, will help enable productive conversations about how we can work together to improve lives by changing systems.
