Read the strategic planThis strategic plan will guide the Connecticut Health Foundation’s work from 2024 through 2028.
The foundation will continue to focus on health equity for people of color, with goals in four areas:
- Expanding health care coverage
- Sustainable funding for community health workers
- Maternal health for people of color
- Fostering the infrastructure for health equity
This document includes a summary of the planning process and why the foundation selected these areas. For each focus area, the foundation identified a long-term vision, a five-year goal, and a set of objectives.
