Grantee Portal Guide

All grant applications must be submitted electronically through the Connecticut Health Foundation’s online grantee portal. Copies of the grant applications as well as instructions for navigating the portal are provided below.

Note: These are provided for review purposes. The foundation will only accept applications submitted through the online grantee portal.

1. Program Grants:

CT Health focuses its funding on two types of program grants – advocacy projects and demonstration models. The foundation funds advocacy projects that seek to change statewide policies to advance health equity as well as demonstration models that help build the case for translating programs into practice. Application questions for these two specific project types can be found below.
