As we reached the midpoint of our 2024-2028 strategic plan, we paused to evaluate and make sure the plan remained on target. As a result of this review, we have made small changes in two of our goal areas. They are:
- Expanding health care coverage: Changes in federal law are likely to increase the number of uninsured state residents in the coming years, so this goal now also includes protecting coverage.
- Maternal health equity for people of color: We have added three new objectives in this area, based off of the findings of our maternal health equity blueprint which was developed in 2025.
This strategic plan will guide the Connecticut Health Foundation’s work through 2028.
The foundation will continue to focus on health equity for people of color, with goals in four areas:
- Expanding health care coverage
- Sustainable funding for community health workers
- Maternal health equity for people of color
- Fostering the infrastructure for health equity
This document includes a summary of the planning process and why the foundation selected these areas. For each focus area, the foundation identified a long-term vision, a five-year goal, and a set of objectives.
